Evergreen Service Providers works closely with the families of Individuals with Intellectual and Developmental Disabilities (IDD) to find a perfect match and place them in a Host Home setting that fosters ongoing growth of mind, body, and spirit. Unfortunately, Individuals with IDDs may be more vulnerable to abuse, scams, and other victimization as well as isolation and loneliness, than those without disabilities. This is a sad fact of which we’re keenly aware. These Individuals may be less likely to report being victimized or to articulate feelings of isolation and need a safe, loving, nurturing environment that enables them to thrive. Evergreen Service Providers is dedicated to making that a reality for Individuals with Intellectual and Developmental Disabilities.

The Role of Host Home Providers
Our Host Home Providers are caring people who open their homes, lives, and hearts to create lasting relationships with the Individuals under their care. Adults with IDDs are matched with Host Home Providers and live in a safe environment that includes a great support system. Host Home Providers not only provide companionship but assist with meeting day-to-day needs and the development of life skills, self-sufficiency, and confidence. The IDD Individual is welcomed into the family and included in meals, activities and events. Host Home Providers help monitor an Individual’s activities, which serves as a safety net to avoid circumstances that may present risks of being victimized.
Community Engagement
Evergreen Service Providers works closely with local Community Centered Boards and agencies that are dedicated to serving the adult IDD population to ensure they have access to all that is available to them through these organizations. We share information with our Host Home Providers about community enrichment and educational programs that are of interest and benefit to each Individual we serve.
Our Services
Evergreen Service Providers meets the needs of each IDD adult by matching them with the ideal Host Home situation. We strive to have a wide geographic range of Providers available to accommodate an Individual’s day program or work location. We have staff on call 24 hours, 7 days a week. A nurse is available as are special diet accommodations and medication management services. We also conduct regular monitoring of the Host Home in which each IDD Individual lives to review safety plans including a fire evacuation plan, and check that working smoke detectors and carbon monoxide detectors are in place. It is our primary focus to ensure the Individual is in an environment where they can succeed and that there are appropriate checks and balances in place to maintain their safety. You can have confidence not only in the safety net we provide for your loved one but our pledge to provide an environment of dignity, stability, and growth in which each Individual can thrive.
