Position Openings:

Host Home Provider

Host Home Providers provide primary residential care and support, in their own home, for adults with Intellectual and developmental disabilities. Along with providing residential services, responsibilities include:

  • Providing meals
  • Transportation to and from appointments and other activities
  • Assisting with daily living skills
  • Medication management
  • Behavioral support in conjunction with an interdisciplinary team

We look for people with big hearts. You’ll provide companionship and assistance in order to help that Individual grow and thrive. The first step in becoming a Host Home Provider is filling out an application. You must also:

  • Pass a Criminal Background check
  • Must be 21 or over
  • Live in a home that meets HUD and State standards
  • Successfully complete the Basic Training Course and First Aid/CPR
  • Complete QMAP certification
  • Maintain professional liability insurance and automobile liability insurance as required by law
  • Understand what it means to be an Independent Contractor/Host Home Provider
  • Understand and follow state regulations around providing services as an Independent Contractor/Host Home Provider

Part Time Nurse (LPN)

Do you get satisfaction from helping people with disabilities? Are you a stay-at-home parent looking for a flexible schedule, or want to fill some extra time? This may be the perfect nursing opportunity for you! Evergreen Service Providers is seeking a part time (20 hours per week) Nurse (LPN) with a kind, professional approach, who wants to be part of a caring, dedicated team in supporting people with developmental disabilities (MR).

Monitor, assess, and address health and medical needs of people supported through our Program in a professional, person-centered, and timely manner to enhance the quality of life for people with Intellectual and Developmental Disabilities. No direct patient care is required.


  • Associate degree in Nursing is required.
  • Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) is required.
  • Maintaining a current Colorado license to practice nursing is required.
  • Employment is contingent on the results of a required background check.
  • Previous experience in providing care for persons with developmental disabilities is preferred.
