Position Openings:
Host Home Provider
Host Home Providers provide primary residential care and support, in their own home, for adults with Intellectual and developmental
disabilities. Along with providing residential services, responsibilities include:
- Providing meals
- Transportation to and from appointments and other activities
- Assisting with daily living skills
- Medication management
- Behavioral support in conjunction with an interdisciplinary team
We look for people with big hearts. You’ll provide companionship and assistance in order to help that Individual grow and thrive. The first step in becoming a Host Home Provider is filling out an application. You must also:
- Pass a Criminal Background check
- Must be 21 or over
- Live in a home that meets HUD and State standards
- Successfully complete the Basic Training Course and First Aid/CPR
- Complete QMAP certification
- Maintain professional liability insurance and automobile liability insurance as required by law
- Understand what it means to be an Independent Contractor/Host Home Provider
- Understand and follow state regulations around providing services as an Independent Contractor/Host Home Provider

CNA / LPN / RN (Part-Time)
We are seeking a detail-oriented individual to manage medical documentation and appointment coordination for adults with developmental disabilities. This is a non-clinical, administrative oversight role focused on ensuring our residents receive high-quality, organized care.
Key Responsibilities:
- MAR Management: Oversee Medication Administration Records, including updating, adding, or discontinuing medications to ensure
100% accuracy. - Appointment Tracking: Schedule and track all medical, dental, and psychiatric visits; ensure all post-appointment paperwork is collected and filed.
- Provider Liaison: Act as the primary contact for medical offices via phone, fax, and portals to request orders or clarify instructions.
- Caregiver Support: Communicate quarterly or as needed with residential caregivers for medication counts, incident report reviews, etc.
- Emergency Guidance: Direct caregivers to appropriate levels of care (Urgent Care vs. ER) during medical situations.
Schedule & On-Call:
- Part-time hours (Schedule flexible/to be discussed).
- On-Call: Availability for up to 2 hours weekly for urgent medication changes or questions.
Qualifications:
- Strong organizational skills and attention to detail.
- Excellent communication skills for interacting with medical professionals and caregivers.
- Experience with MARs or developmental disability services is a plus.
