Greetings!

A season of color and renewal, spring is a reminder of the beauty of individuality and diversity. It’s a perfect time to renew our commitment to and respect for the right of each person to live authentically in an environment that provides them the opportunity to thrive.

You have likely heard about changes related to restructuring the Case Management departments in current CCBs. Rest assured that we are tuned in to these changes and will let you know when there is news.

Evergreen Service Providers is unique because we’re a dedicated, close knit, residential program that feels like family, which makes us a great fit for certain Individuals. We have continued to hone our expertise in navigating the red tape of waivers and our in-depth knowledge of residential options, including the Family Caregiver alternative. Our SCC program, which enables fine tuning activities for each Individual, is proving to be highly effective.

Disability Through a Different Lens

We have seen Supported Community Connection activities, when tailored to the needs and wants of Individuals, make a remarkable difference in their lives. Ethan, a member of our Evergreen family, acts as his uncle’s SCC Coordinator.

Evergreen Service Providers Focuses on Enrichment and Expands its Reach

Evergreen’s focus in 2024 is on enrichment – for Individuals, Host Home Providers, Family Caregivers, and the IDD community as a whole.

Requests for host homes have increased and therefore our need for additional Host Home Providers, especially in the Boulder County area. If you know anyone who might be a great fit for Evergreen’s residential program, please complete a referral form or share our Host Home Providers FAQ webpage with them.
